Adding Subscribe Now & Payment options to Sign-up Process

I am having an issue as my registration/sign-up page doesn't look like the 4th screenshot on this page http://premium.wpmudev.org/project/membership/screenshots/. Neither does it go to a second registration page like it does on WPMU after one signs up. I have the basic sign-up page (attached). How do I get it to reflect the subscription options at sign-up? I have a Free and Premium subscription option.

Instead of seeing the "Complete Sign Up" button, what should the button show if there was a second page where subscription options were?

This is vexing me. Please help. Thanks.

Just checking, is that the custom page with the membership registration shortcode on?

Please advise and perhaps include screenshots of your settings page and ensure all levels and plans are active.

All of my settings and levels are correct. My registration page only has the subscriptionform shortcode and the signup works properly. Its the second step, subscription option that doesnt work as seen in screenshot 4 on the above link. How exactly do I get the subscription option upon signup?

*bump* I don't know if I'm not using the correct terms or phrasing my question correctly, but I'd really like to see Step 2 of the Registration process appear correctly (screenshot attached) as seen in the attached. Step 1: Sign up works properly but the option for a member to chose a subscription plan doesn't work.

Right so in the admin you set the registration page, and its that registration page you are using on the front end? Not the default WordPress one no?

I've just tested and it works as expected for me, which is why I ask.

So if you created a page:

domain.com/sign-me-up/ and thats where the shortcode is, is it that page you are signup up through?
