Sounds like you might have an older version of avatars sitting around there.... can you check your /wp-content/plugins directory for copies of the plugin please? Also check mu-plugins if it exists...

checked. The mu-plugins/avatars.php has been deleted long ago after I have installed the Avatars. What should I do now? Thank you for looking into it.

I do not have the avatars.php in the mu-plugins and wp-content but I do have "Avatars" folder in "wp-content\plugins".

Just to check; are you using WordPress Multisite and do you have the plugin network activated?
Can you just try de-activating and re-activating the plugin please?

Can you please check your "wp-content" directory? The error you're receiving indicates that there is "avatars.php" file present there.

Wow..you are right ! There is a copy of avatars.php in wp-content folder. Deleted & will monitor the log. Thank you.
