We need a way to customize Pay To Blog so that our clients who sign up always have access to their site, once they've paid the "startup fee". I'd assumed that Pay to Blog is the best solution for this...but need the monthly/annual/recurring features removed. Possible?

I believe that would require an extensive customization of Pay to Blog simply because it's subscription based. I imagine you might be able to increase the subscription interval to a very high number to emulate a one-off fee, but I'm really not sure of that, it's something you may want to take a look at posting to the job board.
That said, this plugin as well as the Supporter plugin are due up for some pretty nice changes and I imagine it'll be much simpler to achieve such things once that's released.
As another possible solution, the Membership plugin has a Blog Creation rule that can be used to limit the number of sites able to be created. With its relatively new PayPal with Single Payments gateway you could easily achieve a one-off fee.
Sure thing, William. Basically, Membership has a PayPal with Single Payments gateway that can be used to achieve a one-off payment. Basically, the gateway doesn't rely on PayPal for the subscription, but Membership takes care of that internally.
With that, you could set the subscription interval to whatever you like, including an indefinite duration, so it ends up being a one-off payment. Here's the project page for Membership. http://premium.wpmudev.org/project/membership
Thanks for the discussion here guys.....Hmm.....scratching my head a bit here. I'd prefer to keep things as simple as possible and was drawn to the pay-to-blog solution for that reason. Any timeline re: when these plugins might see updates? At this point I'm leaning towards a bit of a hack on pay-to-blog while keeping an eye on Supporter's updates.....

On the other hand, I'd also want the Supporter feature to control premium themes and plugins.
So, what I did was I use the one-time payment option from Supporter and set the Bulk Upgrade to "very high" (equates to unlimited) number of blogs. So this solve my issue. Hope I'm clear about this.
We can't give an ETA on Supporter but it is in the works.
I should have mentioned also that Supporter actually has a non-recurring fee option so you don't have to set it up for subscriptions. It's also possible to remove other options and just have a single (let's say lifetime) option.
Supporter can be setup to very much mimic the way that Pay-to-Blog works as well. So perhaps that would be a viable option, quiver?

For limiting the amount of options available on the front-end (ie. to provide only one payment option) you can take a look at the following thread where there's the necessary code posted.
