For some reason, when I go to add a user, I do not get an error message and I do not get a confirmation message. The screen refreshes, but the information for the name and email address is still in the fields. I have no idea where to start on this.

I am able to add users through the Network Admin but not on the individual blog. After I added the user to the Network Admin, I can't add them to the individual blog.
When I disable the User Role Editor plugin and I add the user, it appears to add them because it takes a while to refresh (as if it's churning code) and then the information disappears from the form, but the user is not added to the blog.

Yes, I forgot to update the status. I disabled all plugins and added the user, but interestingly enough, she got an error that involved googleanalytics by Joost. So, I re-enabled plugins except that one and then added the user successfully. This was the problem. Thanks for your help and followup.

Hiya!
The first thing you should do is to disable all plugins and revert to the default theme. It's likely that one of them is causing a conflict.
Can you also tell me exactly how you are adding a user in the sub-site admin? Perhaps you can provide a screenshot?
Thanks,
Phil
