I like the form Tammie but it appears that the name field is actually suppose to be the email field.
I get error validation error "Please enter a valid e-mail address." but if I enter email in name field no problem. No email field in the form.
The captcha do not refresh after sending email and doesn't have refresh option prior to sending. I am guessing I need to configure that including thank you, email confirmation etc.
@drdave: Hmm what vesion of wordpress are you on? Have you removed all plugins and tried along with clearing your cache to see if it resolves using that?
@William: Yes it only works in a widget area BUT you can have a widget area on a page.
@drmike: All this does is give you a contact form as a widget. It's powerful though in that sense as you can literally put anywhere you have a widget area. It also doesn't add to the post edit bar like cforms / require advanced configuration / confuse with a ton of options. This basically is the 'very basic you'd need' type of form but sometimes that just is better. Because you can change the fields you could for instance have it not just as a contact form but as a 'suggest a post' or something similar.

You do have a space between the 2 captcha words - I do see how this could be an issue but for now that's the way it works.

@drdave: Yes don't use a space for this captcha in this case. Try adding an email under the widget configuration.

btw, if that's recaptcha, I believe their tos says you have to leave their link in unless you get permission to remove it.

@Lance: It's just put in your plugin folder and activate that's all it's a very simple plugin. You then just use like any other widget.
