Will WordFence work will Multi-DB? If yes, do I install WordFence normally? I am using 16 databases.

Hey there.
I don't see why not from looking at their site, providing it uses WordPress functions to do its business. But unfortunately the only way to know for sure would be to test it out.
I've not tried the two together so I can't say for sure.
Do you have a staging/sandbox site to test with?

I have WordFence installed on two of my WP Multisite installs and it works great. However, if you are running on a shared hosting environment, then make sure that you don't have the auto run site scan feature turned on. WordFence seems to consume a lot of server resources and budget hosts like Hostgator don't appreciate huge resource consuming scripts running frequently, if at all. The initial install and scan should be sufficient. Then you can manually scan every so often. Hope that helps.

I had to move it from shared hosting to VPS but I was wondering if it will work with Multi-DB. Are you using Multi-DB?

Sorry, I missed the original reference to multi-db. I haven't used Multi-DB on my Multisites yet, but I can imagine that WordFence would have any issues with it. As far as I know WordFence checks your file system and such and doesn't really have anything to do with your DB other than say, logging.

Thanks. I may give it a try. If I have a problem, I can always have my hosting company restore a backup.
