Additional fields in Appointments +

I've just purchases a subscription to Appointments+ which seems almost exactly what I need.

I am using in on an advice website where people will be able to book video appointments. This will be quite high profile and will probably generate quite a bit of publicity.

I need to customise the booking form. I have activated "Additional fields" but it doesn't quite give me the functionality that I need (its almost does).

I want to be able to add a heading (what is your appointment about) together with 3 tick boxes (I can add this) and finally a text area field, where they can say a little about their concerns. Do you know if this is easily possible or even possible with a hack.
