[Appointments+] Need Documentation for Add-on

In Appointments+, I clicked on an Add-on called Additional Fields to activate it. However, I can't find where I can add additional fields to the form. Does the Add-On have documentation or can you provide the steps to add addiitional fields to the form that is presented to the user?

    In order to add additional fields to appointment booking form, please follow these steps:

    1. Go to the "Appointments -> Settings [page] -> Add-ons [tab]"
    2. Find the "Additional fields" add-on on the list and activate it

    3. Then go to the "Appointments -> Settings [page] -> General [tab] -> Accessibility [tab]"
    4. At the bottom of the same page the "Additional Fields" section should appear and you should now be able to add your fields there
