Appointments+ not accepting new appointments

The appointments+ page seems to be loading, but is not responding to a request for a new appointment. When I look at the page on hartcenter.com in developer mode, the error is "Failed to load resource: the server responded with a status of 400 ( )

    This seems like some kind of conflict with some other plugin or theme. Could you please proceed with a conflict test as shown in this flow chart?

    First, please have a complete backup in place (files and database), in case you lose any other settings during this test, so you have to restore it.
    Then deactivate all other plugins apart from WPMUDEV Dashboard and Appointments+, activate a default theme like TwentySeventeen, deactivate all caches and try to book a new appointment.
    If this is resolved, activate the remaining plugins and theme, one by one this time, and try to book a new appointment each time, until you reproduce the initial issue again.
