Appointments+ not saving appointments or sending emails

I have set up Appointments+ but it is not saving the appointments for the provider or sending the confirmation email or the email to the provider or to the google calendar. It shows that the time slot is not available on the make an appointment page but that is the only indication that there has been an appointment made. I have activated the support access so you can see how it is all set up. I've double checked all the settin and I can't figure out why this isn't working.
