    Yes. I've tried all that you suggested.

    It worked before upgrading to wp 4.2.1 although it was quite buggy (squished logo image etc), but now it simply doesn't work. A closer look at the generated CSS shows that although the image does get uploaded and the CSS gets generated, the original wordpress logo supersedes and changes. It could be a change in 4.2.1 that resulted in this.

    Here's the code from the Inspector.

    body.login div#login h1 a {
      background-image: url(http://yourdomain.com/wp-admin/images/wordpress-logo.png);
      padding-bottom: 30px;
    }
    .login h1 a {
      background-image: url("http://yourdomain.com/wp-content/uploads/2015/05/my-network-logo.png");
      background-size: 300px 86px;
      background-position: center top;
      background-repeat: no-repeat;
      color: rgb(153, 153, 153);
      height: 86px;
      font-size: 20px;
      font-weight: 400;
      line-height: 1.3em;
      margin: 0px auto 25px;
      padding: 0px;
      text-decoration: none;
      width: 300px;
      text-indent: -9999px;
      outline: 0px none;
      overflow: hidden;
      display: block;
    }
