Change form layout of a pop-up

I’m trying to change the style of a pop-up. I would like to place an image on the LEFT with a button on it and, on the RIGHT I would like to have:

Some text + the form fields (one on top of each other) + another button below them + a Facebook share button + some text below it.

    I have created a form with a similar display such as the one you had shared with us. I went ahead and setup the popup to display (temporarily) on the current contact page for you to view it: https://nu****re.coach/contact/

    I was able to create the popup applying similar design by using the following custom css:

    .hustle-modal .hustle-modal-optin_form {
    display:block;
    }.hustle-modal .hustle-modal-close .hustle-icon {
    margin-top:0px;
    position:absolute;
    }.hustle-modal .hustle-modal-body {
    height:550px;
    margin-top:-30px;
    }
    span.hustle-modal-optin_placeholder {
    text-align: center;
    }

    You can find the code by editing the popup and going to Design -> Custom CSS field
