    It seems that you need to embed a Zoho Form in your popup. First, in order to be able to render this form, you should install the following plugin:

    https://wordpress.org/plugins/zoho-forms/

    This will give you the ability to use a shortcode like [zohoForms] in the content of your popup. To be exact, as I already tested that, this should do:
