Comments Plus Issue with double entry area, Facebook not logging out, and CSS Question

I seem to have two comment input areas showing up. How do I just keep the bottom one with the comments plus area?

I also want to know where the CSS is for the plugin so I can change the way it looks. Can I just add a new theme with my own css here:
comments-plus/css/themes/put_new_theme_name_here.css

I can't get the plugin to disconnect from Facebook either. The disconnect button doesn't work.

    Sounds like something in the theme which our code hooks into twice. If its a free theme or one I have access to then I can look at that for you.

    Please provide a link to where I can grab the theme.

    If not then you can create a manual hook and place that in the code instead.

    You can create a custom hook in the admin:

    Admin --> Network Admin (or not if in single site mode) --> Settings --> Comments Plus

    /wp-admin/network/settings.php?page=wdcp

    You can style directly within your theme and turn off the plugin styling.

    Updating this theme is not super easy, but doesn't actually change much (after looking at the change logs from the two updates) other than security. I can't seem to figure out where in the code the comment hooks are located.
