Enewsletter plugin not showing form fields

I've tried many configurations of WordPress hosting, themes and plugins, including a brand-new installation, and enewsletter has the same symptom each time - it shows only the Widget Title and an Unsubscribe link (with no url), and no form fields at all. I can't seem to find a plugin conflict at all, or a good reason why it isn't working. I do see a lot of code in the source, that doesn't seems to be displaying. Help!
Here are three live pages with the plugin not working:

    Hey Gregg,

    Hope you're doing well today!

eNewsletter will only show form fields for visitors(logged out) as you can see in my screenshot. I see fields on all 3 of those pages.

Logged in users simply need to click "subscribe" or if they are already subscribed then there will be an unsubscribe link, which is what you are seeing. Although, it should be linked so you can actually click it to trigger the unsubscribe.

Can you grant support access? Then we can have a closer look.

You can grant support access via WPMU DEV > Support > Support Access > Grant Access.

    Look forward to hearing back!

    Cheers,
    Tyler

Browsers very unpredictable for this plugin. I restarted Firefox for Mac, and it suddenly started showing the form fields properly on all three page examples. But Safari for Mac is not showing 1 of the 3. (http://wordpress-10404-23045-55857.cloudwaysapps.com/)

Confused. I'd also like to be able to choose more fields to populate new user accounts and signups, EG: First Name and Last Name separately.

Tyler! Thanks, the fact that I was logged-in sometimes was the problem. If someone signs up for the newsletter, but doesn't have a user account in the first place, I suppose they will always see the fields, because they can't be logged-in?

Also, the Unsubscribe link is working now that I know it's an inline message, not a new page. Thanks for your help.

I did have another request, and that is to be able to control the fields that are displayed and how they are labeled and ordered. Can I change those things? Can I have the plugin store First and Last Name separately?

That's correct, they will keep seeing the fields unless they do make an account later on with that email and are logged in with it.

I did have another request, and that is to be able to control the fields that are displayed and how they are labeled and ordered. Can I change those things? Can I have the plugin store First and Last Name separately?

Right now enewsletter only allows for a single name field and email; however, I'll make this a feature request so we can hopefully add in the option in the future.

As for the label, that we can change

So perhaps make the name field label "First and Last Name" for now?

Here is how to change the name field.

Use our Ultimate Branding plugin and activate the "network wide text change" module. Then fill it out as you see in my screenshot.

The text domain field is to make sure it only changes that text within eNewsletter. That way it won't change text that you don't want changed.

Hope that helps! If you have further questions just let us know.

    Cheers,
    Tyler
