Forminator error messages and won't submit form

I created a form using Forminator and included it in a post for my customers to complete their annual registration and when I tested it, my submissions were logged and viewable - however, now that I've posted it and my users are trying to submit them to me there are problems that I can't find the cause. My users are getting error messages and none of the forms have been saved in the dashboard.
The form is located at if you want to look at it and maybe offer a solution on how to remedy the problem.

    Hello Kelley,

    I'm sorry to hear about this issue with our plugin.

    I can see there is a problem with data validation when the form is submitted. But this might be a problem with misconfigured Asset Optimization in Hummingbird because I can see lots of JS error on that page.

    Can you disable Asset Optimization for a moment and see if there will be change with submitting the form?
    (you might want to create a backup of the site before doing any changes)
