    Hello Jerome,
    I hope you are doing well.

    From reviewing your site I figured out that this style (display: none) has added to your form which made this issue. To solve this issue you need to add this code to your stylesheet (Go to WordPress admin panel Appearance > Customize > Additional CSS):

    #forminator-module-534
    {
        display:block !important
    }

    Also, I find that there is still missing some styling. So would you please enable support access to have a closer look on your site? Please follow this article to enable support access: https://premium.wpmudev.org/docs/getting-started/getting-support/#chapter-5

    Regards,
    Ahmed

    Hello Jerome,
    I hope you are doing well.

    From accessing your site, I created a new form to check if this issue is related only to your form or not. I figured out the issue is still occurred with my new form.
    To diagnose this issue, could you please do a plugin conflict test?

    Note: Before proceeding with the conflict test, I strongly suggest having a backup.

    To do the conflict test follow these steps:
    1-Login to your site via FTP
    2-Go to wp-content
    3-Rename the plugin folder to _plugins.
    4-Create a new empty plugins folder.
    5-Move the plugin folder (Forminator) you want to test from _plugins to plugins folder
    6-Check if the issue is still happening, if no, continue to the next step.
    7-Then, one at a time, move the other plugin folder from _plugins to plugins folder until the issue appears again.

    Note: While you are doing this test don’t visit WP Admin > Plugins on your site. This will ensure that your plugins remain active when you copy them over.

    Let me know the results.

    Regards,
    Ahmed

    Hello Jerome,
    I hope you are doing well.

    I have noticed that you are using a custom theme and maybe that is the reason for that issue so could you please try to install one of a default WP theme (ex: twentyseventeent) and repeat the conflict test?

    Let me know the result.

    Regards,
    Ahmed.

    Hi,

    Even if this works (It might), I will be stuck because doing another theme is out of question.

    Which functions do you use to inject the js and css in the theme?
    I will make sure they are on our custom theme.

    THanks

    Hello Jerome,
    I hope you are doing well.

    To inject CSS stylesheet and JS to your theme, you should write the following code in wp-content/themes/your-theme-folder/functions.php file and you should write it at the end of the file to override any other style:

    function any_name(){
         wp_enqueue_style(“any_name” , get_template_directory_uri().”/css/your_stylesheet.css”);
         wp_enqueue_script(“choose_anyname”  , get_template_directory_uri().”/js/name_of_script.js” , array() , “1.0.0” , true);
    }
    add_action("wp_enqueue_scripts","any_name");

    If you want to know more information about functions I use, for enqueueing stylesheets and scripts https://codex.wordpress.org/Plugin_API/Action_Reference/wp_enqueue_scripts and getting the path of active parent theme https://developer.wordpress.org/reference/functions/get_template_directory_uri/

    Regards,
    Ahmed.
