How to add extra fields in appointments+

I am setting up Appointments+ for a customer who is a photographer of houses for sale, To book an appointment obviously the customer must choose service and service provider and then book a time and date. So far so good, BUT I need to have some field for the customer to put the PLACE (address) for the job, and also additional information that might be needed for the booking (phonenumber to the address, code to door, et.c.) How can I make this happen?
