I installed and activated Q&A plugin as instructed

I installed and activated Q&A plugin as instructed on the site. When I go to: http://mysite.com/questions/, the page shows up. When I try to add questions by going to: http://www.mysite.com/questions/ask, I get a "Page not found" error. Is there something special I need to do to make this page show up so? (I've already tried switching to the WP default theme and get the same result)

    I installed and activated Q&A plugin too. My pages just look so bad -- and the ask a question page is unusable.

    I have 2 issues:
    #1: http://askcamperrick.com/questions/ask/
    My "ask a question" page is totally unformatted so that it is unusable. How can I get a template applied to that page so it will work?

    #2: http://askcamperrick.com/questions/info-included/
    This page is a question I added through the back end. There is supposed to be some content in the R nav area ... and it's laying below the Q&A content. Again, I think this is a template issue.

    I'd appreciate some help!

    Can you take a read through this and try this. This is taken directly from the usage tab of the plugin's project page.

    Customisation Options:

    Optionally, you can further customize the appearance of the Q&A section, to better integrate it with your theme.

    To do so, simply copy the php files from default-templates into your theme folder and start customizing.

    If you want to disable the default plugin CSS, add the following line to your theme’s functions.php file:

    add_theme_support( ‘qa_style’ );

    If you also want to disable the default JavaScript (not recommended), add the following line to your theme’s functions.php file:

    add_theme_support( ‘qa_script’ );

    CUSTOMIZE YOUR ROOT SLUG

    If you want to change the Slug, or url, of your Questions you can define any of the following in your wp-config.php

    QA_SLUG_ROOT

    QA_SLUG_ASK

    QA_SLUG_EDIT

    QA_SLUG_UNANSWERED

    QA_SLUG_TAGS

    QA_SLUG_CATEGORIES

    QA_SLUG_USER

    using the following format define(‘QA_SLUG_ROOT’, ‘yourslughere’);
