membership pro forms, titles and selected boxes

I am using membership pro with buddypress.
I have one form tickbox question where it is a collection of options of which only one can be ticked. Currently they can all be ticked. You can see the form here : http://www.mdnbeta.org/apply/?action=registeruser&subscription=2
Firstly I wanted only the overview text on the first page, not above the form and secondly is is the Subscription (required) question that I want only one box clickable - how would I do this?

    You would need to switch the field type to radio buttons, checkboxes are designed to multi-select out of the box (it'll be like this with only BuddyPress enabled, tested on my own site quick).

    You may wish to try inserting this CSS code into your site, using a plugin like Simple Custom CSS:

    .page-id-15.registration .post-content > p {
        display: none;
    }
    
    .page-id-15.logged-in .post-content > p {
        display: none;
    }
