Permalinks resetting erratically and causing 404

I am running WPMU 2.9.1 with BuddyPress. The remove permalink menu item WPMU plugin is installed. For some reason, after a user posts, the permalinks on the blog become corrupted and all subpages on the blog result in 404 errors. Not all blogs are affected, just random ones. This does not happen after every post, just occasionally.

The fix is to go in an hit save on the option-permalink page, but after a few days or so, a post will corrupt the permalinks and the site sub pages will result in 404.

The site is set up as sub folders : http://www.example.tld/community/bloghere

Does anyone know why permalinks would become corrupted?
